Payroll Deduction Request

If your company or organization has Payroll Deduction set up with Lambert's Uniforms please complete the form below. Lambert's will contact you via email as soon as your employer approves the Payroll Deduction Request. This process can take up to 7 days depending on your employer. Please note that some hosptial payroll accounts don't allow web store payroll deductions, only in-store purchases.
